What is Needed for Enrollment
To register, please turn in the registration form along with the $100 registration fee to the Director at 61 Ritchie Hwy.

Jan 2nd for Current Families

Jan 10th for Alumni and PUMC members

Jan 17th for New Families
To complete the enrollment process, the tuition deposit and emergency form must be turned in by May 1st.

Health forms (Health Inventory, Immunizations, and Blood Lead Test) are also due by May 1st for students who have birthdays between Sept 2nd - April 30th.

For students who have birthdays between May 1st and Sept 1st, please turn in the medical forms after your child's next wellness appointment.

All medical forms must be turned in prior to your child's first day of school.
For Students with Medical Conditions
If your child has a medical condition, additional forms are needed and must completed by your child's doctor prior to their first day of school. Please contact the Director to find out which forms are required.
